Jeffrey Warren "Jeff" Holtzclaw, age 33, of Radcliff, formerly of Lebanon, passed away Tuesday, November 13, 2018 in Hodgenville, Kentucky.
He was born on November 24, 1984 in Campbellsville, Kentucky.

He is preceded in death by his maternal grandfather, Lambert Livers; paternal grandparents, Albert and Jewell Holtzclaw; uncle, Kenny Livers; partner, Joseph Bell.

Survivors include his daughter, Elizabeth Dawn Holtzclaw; son, Lucas Warren Holtzclaw; mother, Margie Livers Holtzclaw of Lebanon; father, Joey Holtzclaw of Campbellsville; brother, Elijah Livers of Lebanon; two sisters, Alexis Livers and Olivia Livers, both of Lebanon; maternal grandmother, Agnes Deverial Livers of Lebanon; three aunts, Theresa (Keith) Richerson, Cheryl (Terry) Yates, Jackie Livers; four uncles, Joe Livers, Walter (Lydia) Livers, Jimmy Livers, Chris Livers; paternal uncle, Mark (Sherri) Holtzclaw.

Funeral Services will be held at 1:00 P.M. Saturday November 17, 2018 at the Open Arms Community Church. Interment to follow in Our Lady of the Hills Church Cemetery.

Visitation will be held from 4:00-8:30 P.M. Friday November 16, 2018 at Bosley Funeral Home, and on Saturday November 17, 2018 from 8:00 A.M. until the time of service at Open Arms Community Church.
